Prison patients are never alone and never without supervision and rules, and the medical staff is always negotiating its power with the adminsitration. The patient-doctor relationship can become distorted in this setting.
This article examines conceptual limitations of extant accounts of palliative psychiatry, with a focus on obligations to distinguish among and clearly formulate goals of care.

Prognostication means informing patients what the outcome of their illnesses and treatments are expected to be, based on the best available evidence. Prognosticating in a way that helps patients in decision making about treatment is difficult for physicians to do.
